Midsole foam compresses over months of daily loading, and once it has flattened the boot stops doing the thing you bought it for even though the upper and outsole still look fine. If your boots feel dead underfoot, that is the replacement signal rather than waiting for visible wear.

On flat hard ground, generally yes, and the two work together rather than competing. An aftermarket insole helps and is worth trying, though it works on the footbed rather than the midsole, so it cannot replicate a properly engineered midsole. If your boots have a removable footbed and are otherwise good, a quality insole is a cheap improvement. If the midsole itself has gone flat, no insole will fix it.

It matters a great deal, and the two are separate levers. Weight on your feet costs disproportionately more energy than weight elsewhere, because you lift it thousands of times a day.
